D. REQUIREMENTS FOR MINOR FIRST-TIME APPLICANT (below 18 years old)

Minors applying for a passport should be accompanied by the parent/s or legal guardian. If the child is born in the United States, the birth must be reported first to the Philippine Embassy/Consulate General having jurisdiction over the place of birth of the child. 

Confirmed passport appointment and and personal appearance of the minor applicant and accompanied by:

a. The mother – if parents are not married;
b. Either parent – if parents are married; or
c. Legal guardian – (please see No. 6 supporting requirements below)

Present an original and photocopy of the Birth Certificate/Report of Birth of the minor/child to show filial relationship between the minor child and accompanying parent:

  • If born in the Philippines – issued by the Philippine Statistics Authority (PSA) formerly National Statistics Office (NSO). Additional document/s may be requested if the BC/ROB presented is unreadable
  • If born abroad – PSA authenticated ROB. Those born in the United States and within the jurisdiction of the Philippine Consulate General in San Francisco may present the original ROB if the PSA-ROB is not yet available
  • Proof of Parental Authority or Filiation – the following documents may be presented in lieu of the minor’s Birth Certificate as long as the full name of parents are shown and filial relationship is established:
    (a) Baptismal Certificate
    (b) Baby Book
    (c) School Records
    (d)
    Copy of Mother’s passport (surname and middle name must be similar to the minor applicant)

Present the original and photocopy of the following:

    • Valid Resident Alien Card/ Permanent Resident Card;
    • If the applicant is a DUAL Citizen – present the original and photocopy of dual citizenship documents; or
      – Identification Certificate
      – Oath of Allegiance
      – Order of Approval
    • Copy of Valid U.S. Visa (if tourist, leisure, or business purpose).

Original and photocopy of the Passport and valid government-issued ID of either Parent or Legal Guardian.

A. The following supporting documents will be required for the following cases for Minor New/Renewal Passport Application:

1. In case applicant is accompanied by only one (1) parent during the application process:

    • If parents are married – present an original and photocopy of the Marriage Certificate
    • If parents are not married and minor is accompanied by the Father – a Notarized Affidavit of Support and Consent with Special Power of Attorney executed by the Mother with a copy of mother’s valid ID and/or passport is required. If the Affidavit is executed in the Philippines, it must be apostilled by the Department of Foreign Affairs (DFA). If it is executed outside the Philippines, it must be:
      (a) Apostilled by the issuing State or Competent Authority if it was issued in Apostille countries; or
      (b) Authenticated/Acknowledged by the Philippine Embassy/ Consulate General if it was issued in non-Apostille countries.
    • Court Order awarding guardianship or parental authority, if parents are divorced or annulled. 

2. In case applicant is accompanied by a Legal Guardian during the application process:

    • Documents showing legal guardianship.
    • Original and photocopy of the Passport or valid government-issued ID of the legal guardian.
    • Present a Notarized Affidavit of Support and Consent with Special Power of Attorney executed by the Parent/s with a copy of their valid ID and/or passport is required. If the Affidavit is executed in the Philippines, it must be apostilled by the Department of Foreign Affairs (DFA). If it is executed outside the Philippines, it must be:
      (a) Apostilled by the issuing State or Competent Authority if it was issued in Apostille countries; or
      (b) Authenticated/ Acknowledged by the Philippine Embassy/ Consulate General if it was issued in non-Apostille countries.

3. In case applicant is not accompanied by the parent/s during the application process:

    • Applicant must submit a Notarized Special Power of Attorney executed by the Parent/s with a copy of their valid ID and/or passport. If the SPA is executed in the Philippines, it must be apostilled by the Department of Foreign Affairs (DFA). If it is executed outside the Philippines, it must be:
      (a) Apostilled by the issuing State or Competent Authority if it was issued in Apostille countries; or
      (b)
      Authenticated/ Acknowledged by the Philippine Embassy/ Consulate General if it was issued in non-Apostille countries.
    • Original and photocopy of the Passport or valid government issued ID of authorized adult companion.

4. In case applicant is an illegitimate child and the Mother is deceased/absent and the Father is unknown:

    • PSA Birth Certificate
    • Notarized Affidavit of Guardianship
    • Original and Photocopy of passport or valid government-issued ID of adult guardian d. DSWD Clearance 

5. If the minor applicant has undergone the process of domestic adoption:

    • PSA Birth Certificate
    • Valid passport or valid government-issued ID of adoptive parent/s
    • Philippine Court Decree of Adoption and documents to prove the identity of the child (e.g. Birth Certificate, Certificate of Foundling)
    • If applicant is not accompanied by adoptive parent/s during the application process – applicant must submit a Special Power of Attorney executed by the adoptive parent/s designating the minor’s companion to assist in the application process 

6. If the minor applicant is undergoing or has undergone the process of foreign adoption/ Inter Country Adoption Board (ICAB) adoption process:

    • Personal Appearance of minor applicant and ICAB representative
    • PSA Birth Certificate/ PSA Certificate of Foundling
    • Endorsement from ICAB
    • Certificate of Child Available for Adoption/ Deed of Voluntary Commitment
    • Placement Authority issued by ICAB
    • Certificate for Issuance of Passport issued by ICAB
    • Clearance for Inter-Country Adoption
    • Child Study Report
    • DSWD Clearance 

8. If the applicant is a foundling and NOT for adoption:

    • PSA Certificate of Foundling
    • Notarized Affidavit of Guardianship
    • Passport or valid government-issued ID of adult guardian
    • DSWD Clearance

B. Certification Requirements (Certified True Copy of Passport)

1. For this transaction, please secure an online appointment for Legal & Notarial Services.
2. Personal Appearance of the Passport holder
3. Original Passport and the number of photocopies needed for CTC – one photocopy will be used for the Consulate’s file

The Philippine Consulate General reserves the right to require additional proof or documents from an applicant, to determine his/her citizenship/identity, the authenticity of the documents submitted, and to ensure accurate and complete personal data entries, pursuant to the Philippine Passport Law.

C. Civil Registry Services provided by the Philippine Statistics Authority:

Certified copies of Birth, Marriage, and Death Certificates, and Certificate of No Marriage Record (CENOMAR), may be requested at PSA Serbilis (www.psaserbilis.com.ph).

Personal checks and credit/debit cards are not accepted.

Please provide a (a) Self-Addressed Stamped Return Envelope with appropriate stamps and tracking number (USPS Express or Priority Mail) or (b) Self-Addressed Prepaid Mailing Envelope from courier of choice (do NOT use FEDEX) labeled with the recipient’s address. As a reminder, please do not put the name of the Philippine Consulate as the sender, should the Shipping Label be “to Bill the Sender”.

You may track the status of your passport at Passport Tracking System.  Input the tracking number that will be indicated on your claim stub.

Note: The Philippine Consulate General assumes no responsibility for any delay or loss in the mail, or while the documents are in the custody of the courier service. The applicant should note the tracking numbers of all envelopes used and submitted, and may track their envelopes at the website of their chosen courier.

The applicant may (a) surrender OR (b) mail to the Philippine Consulate his/her valid passport before the Consulate can release and mail the new passport to the recipient’s address. The valid passport must be cancelled first before the new passport can be released. Both old and new passports will be mailed back to the applicant. If the latest passport being renewed is already expired, it will be returned to the applicant during the passport application process.

Only a Filipino citizen is eligible to acquire a Philippine passport.

Set an appointment. Appear on the actual date of appointment and bring the complete set of requirements appropriate to your application.

Appointment slots open every Monday at 4:30 p.m. at https://www.passport.gov.ph. Only slots for the following week are opened.

Walk-in transactions are permitted, subject to capacity limitations.  However, walk-in applicants may only be accommodated once applicants with appointments are processed.
 
For the convenience and safety of the public, the Consulate recommends setting an appointment for passport at passport.gov.ph.  New appointment slots are uploaded every Monday at 5:30 p.m. (Pacific Tme).

You should expect your passport to be released 4-8 weeks after being accommodated for processing.

In accordance with Department Order No. 010-2017 or the Implementing Rules and Regulations (IRR) of Republic Act No. 10928 (2017), the Philippine passport has a ten-year validity period, for those issued to Filipinos 18 years or older. Filipinos under 18 years will still be issued five-year valid passports.

If your passport details has any discrepancy, you are required to report such inconsistency to the DFA capturing site where you originally filed your application for appropriate action.

No. Your passport must be at least six-months valid for you to be able to travel abroad.

In case of lost or stolen valid passport, you may file and report your lost valid passport application at the Consular Office of your choice. You will be required to submit a Police Report (in English) and an Affidavit of Loss. There will be a 15-day clearing period prior to the processing of application for the replacement of a lost valid passport.

 

In case of lost expired passport, you will be required to submit Affidavit of Loss. Your application will be considered as first-time passport application.

In case of mutilated or damaged passports, you are required to submit Affidavit of Mutilation along with your passport application requirements.

No. The Philippine passport has a standard booklet containing forty-four (44) pages. Should the passport run out of pages, you may opt to renew your passport.

The Department will cancel your passport if you do not claim it within one hundred and eighty (180) days from the application date.

Yes. You may request for a Certificate of Unclaimed Passport from Consulate or the concerned Consular Office, and submit this certification together with your application.

Yes, provided that you have not yet used your married name in any previous Philippine passport. However, you may be required to submit a PSA Marriage Certificate or Report of Marriage (ROM) if you are updating your marital status from “single” to “married”.

You may revert to your maiden name only if marriage is annulled or you are already a widow. Adequate documentary evidence is required.
